REIMBURSEMENT REQUEST FORM
1. Click the button below to open the form. You must download the file to your computer. Once downloaded, the form can be filled out digitally, and your version should be saved to your computer. Please email your saved file to our treasurer. Missing info may delay reimbursement.
2. Provide receipts for all purchases. (Digital images of receipts accepted.)
3. The completed form and receipts may be emailed to firstname.lastname@example.org. Please contact the treasurer if you need to arrange a hard copy submission.
